Adult Day Services gives you the opportunity to work, go to school, volunteer, spend time with other family members, or simply have a break. You can be assured that they too are enjoying their time away from home with this extended family and home away from home.

GracePointe Crossing Adult Day Center provides health and personal care services, as well as invaluable socialization for adults who would otherwise be isolated in their homes. The services support the choice of older adults to remain in their home or with family.

Adult Day Center Services

• Daily social and recreational activities

• Music, art and literature
• Nutritious noon meal and snacks
• Health monitoring by trained staff
• Spiritual care
• Support and tips for healthy living at home

Above all there are plenty of opportunities for smiles and laughter.

Services are available for full or half days at a reasonable per-visit cost, including a meal. Additional fees are applied to extended hours. Financial assistance for qualifying  participants may be available through Isanti County Public Health/Family Services.

Hours

Mondays-Fridays from 9:00 a.m.-3:00 p.m. with extended hours until 6:00 p.m. available as needed.

Transportation

Day center participants living within 15 miles of the center may utilize the transportation service provided by GracePointe Crossing for a standard fee. An additional per-mile fee is applied for participants outside city limits.
